In short

You have 21 days to register a birth in India. Past 30 days it needs an affidavit and written permission. Past a year it needs a magistrate's order.

Two things save the most pain later. Order five certified copies at the start, and check every spelling at the counter before you leave. A correction at three months is an inconvenience. The same correction at fifteen, when the certificate no longer matches school records and Aadhaar, is a multi-year problem.

Registration happens at the place of birth, not where you live. If you delivered in another city, register there. Most corporations now allow online application and postal delivery.

Do not assume the hospital did it. They usually forward the record to the registrar. Usually. Ask for the acknowledgement and follow up in week two. This is the commonest reason parents discover a problem months later.

The three mistakes that cost families years

Leaving the name blank. Many hospitals register Baby of followed by the mother's name. Adding it later is a separate application. If you have decided the name, give it at registration even if the naming ceremony is weeks away.
Ordering one copy. Get five. School admission, passport, Aadhaar, insurance and one spare. They cost 20 to 100 rupees each, and getting them later means another trip.
Not checking spellings at the counter. Check the child's name, both parents' names, the date, the time, the place and the sex before you leave the building.

Fixing an error

Apply to the registrar who issued it, not a different office. Expect an affidavit on stamp paper, and weeks rather than days. Bring originals and photocopies of everything.

Special situations

Born abroad to Indian parents. Register with the Indian Mission or Consulate, ideally within a year.
Adoption. The CARA process reissues the certificate in the adoptive parents' names by court order.
Surrogacy. The Surrogacy Regulation Act, 2021 changed documentation substantially. Get case-specific legal advice.
Single or unmarried mothers. You are entitled to register the birth and obtain all documents. The Supreme Court has held that a single mother can be sole guardian without naming the father. Some officials are unaware of this, so carry a printed copy of the guidance.

What comes next

Bal Aadhaar. Free, blue card, no biometrics under age 5. Set calendar reminders for the 5th and 15th birthdays, because both carry a mandatory biometric update and missing them can deactivate the number.
Health insurance. Most insurers have a 30 to 90 day window to add a newborn. Email HR or your insurer in week one and get the answer in writing.

Passport, only when you are actually travelling. Allow four to eight weeks.
